Job summary

Flagstar Rehab is hiring a licensed Physical Therapist to work in a Skilled Nursing Facility in the NYC area. You will evaluate patients, develop individualized plans, and educate patients and families as part of a multidisciplinary team.

Responsibilities include supervising PTAs, delivering quality therapy across conditions such as stroke, TBI, Parkinson’s, and post-surgical rehab, and documenting progress in our EMR system. Competitive pay and benefits available.

Qualifications

• Current NYS PT license and registration.
• CPR certification.
• EMR competence and comfort with documentation.
• Meets all state and federal requirements for US employment in a nursing facility.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
