Job summary

Flagstar Rehab is staffing Physical Therapists for Skilled Nursing Facilities in New York City and Long Island. PTs perform evaluations, treatments, and patient/family education to improve physical function and cognition, helping daily life activities.

PTs can be employed directly by our agency or operate as independent contractors; corporation setup assistance is provided. Pay rates range from $46–$48 hourly (W-2) or $57–$60 hourly (1099).

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR Competence.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, addressing conditions such as TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using EMR software.

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Long Island.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
