Conduct thorough evaluations and develop individualized treatment plans for residents with a wide range of medical conditions and injuries
Implement and modify physical therapy interventions to promote optimal mobility, strength, flexibility, and function
Provide direct care to residents, including one-on-one therapy sessions, group therapy, and education to residents, families, and caregivers
Document all therapy sessions and progress accurately and in a timely manner
Collaborate with other healthcare professionals to ensure comprehensive care and coordinate efforts to achieve resident goals
Participate in care conferences and interdisciplinary team meetings to discuss resident progress and develop care plans
