Job summary

Walnut Creek Nursing and Rehab in Erie, PA is seeking a Full-Time Physical Therapist Assistant (PTA) to join our interdisciplinary team and help residents reach their healthcare goals.

You will assist licensed physical therapists in developing and implementing plans of care, provide hands-on treatment, assess progress, and collaborate with the care team to ensure seamless, resident-focused rehabilitation and discharge planning.

Qualifications

• Licensed Physical Therapist Assistant (PTA) licensed in Pennsylvania.
• Indirect License preferred.
• SNF/LTC experience is a plus.
• Experience with Net Health is a plus.
• Strong communication and teamwork skills.

Responsibilities

• Assist physical therapists in the development and implementation of patient treatment plans.
• Provide hands-on patient treatment, including exercises, modalities, and other interventions.
• Conduct patient assessments and evaluations to identify areas of improvement.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Participate in patient education and discharge planning.
• Maintain accurate and timely records of patient care and progress.
