Job summary

InHome Therapy is hiring licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around McHenry. If you are looking for a flexible schedule, consistent caseload, and a team that supports you - we would love to hear from you.

Deliver in-home physical therapy services, evaluate mobility, implement rehabilitation programs, educate families on recovery plans, and maintain Medicare-compliant documentation.

Qualifications

• Active physical therapy (PT) license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.
• Basic comfort with technology - email, texting, EMR systems (training provided)

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community.
• Evaluate patient mobility, strength, and function to develop individualized treatment goals.
• Implement effective rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate and timely home health care documentation per Medicare and agency standards

• IHT PT Clinical Excellence Bonus - earn additional weekly pay based on visit complexity and volume
• The evaluation visit rate for this role ranges from $65 - $85 per visit

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
