Job summary

InHome Therapy in Chicago area is seeking licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around South Chicago. If you are looking for a flexible schedule, consistent caseload, and a team that supports you - we would love to hear from you.

You will deliver in-home physical therapy services, evaluate mobility, implement rehabilitation programs, educate patients and families on recovery plans, and maintain Medicare-compliant documentation.

Qualifications

• Active PT license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients.

• IHT PT Clinical Excellence Bonus - earn additional weekly pay based on visit complexity and volume
• The evaluation visit rate for this role ranges from $65 - $85 per visit

Key Responsibilities

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Basic comfort with technology - email, texting, EMR systems (training provided)

This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
