Job summary

Interim HealthCare of West Michigan is seeking a Home Health Physical Therapist (PT) in Kalamazoo, MI to deliver patient-centered therapy at home. You’ll assess mobility, design personalized treatment plans, and guide patients to regain strength and function in their daily lives.

You’ll work with an interdisciplinary team, maintain clear progress documentation, educate families on goals and exercises, and uphold high standards of care. A Michigan PT license and CPR are required.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active PT license in Michigan.
• 1 year of clinical experience, preferably within home health in the last 5 years.
• CPR certification.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients in their homes.
• Assess movements, document progress and modify care plans as needed.
• Assist patient with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on the plan of care and self-care strategies.
• Collaborate with RN/LPN/CNA/HHA/OT/SLP as part of the home health team.
