Job summary

Interim HealthCare Inc. in Naples, FL is seeking a Home Health Physical Therapist (PT) to deliver patient-centered therapy at home. This role offers 1:1 therapist-to-patient care, flexible assignments, and autonomy within a supportive home health team.

You will assess, treat, and educate patients in their homes, collaborating with nurses and aides to reach individualized goals while maintaining compliance with home health regulations.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active PT license in Florida.
• Minimum of 2 years of physical therapy experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functional limitations.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) per plan of care.
• Observe movements, assess progress, and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to manage pain and improve mobility.
• Educate patients on plan of care, goals, exercises and self-care.
• Coach families on the plan of care and therapy goals.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Florida
• Minimum of 2 years of physical therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
