Interim HealthCare of West Michigan is seeking a Home Health Physical Therapist Assistant to deliver personalized in-home physical therapy for patients with mobility challenges in Madison, WI and surrounding areas. You will work within a supportive home health team to advance patients toward independence and improved quality of life.

Responsibilities include supervising therapy sessions, educating patients and families, and documenting progress to refine care plans.

Qualifications

• Must be a Graduate of an accredited Physical Therapist Assistant Program.
• Active PTA license in Wisconsin (WI).
• Strong clinical judgement and compassionate patient care.

Responsabilités

• Provide home-based physical therapy to patients with mobility limitations in their homes.
• Collaborate with a home health team (RN, LPN, CNA, HHA, OT, PT, SLP) to achieve care plan goals.
• Observe movements, document progress, and adjust care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on the plan of care, goals, exercises, and self-care.
• Coach family members on safe assistance with exercises and adherence to the plan of care.
