Interim HealthCare Inc. is seeking a Home Health Physical Therapist Assistant (PTA) in Michigan to support patients in their homes. You will work under a PT, participate in assessments, and contribute to care plans tailored to each patient’s environment and goals.

Qualifications

• Current licensure as a physical therapist assistant in Michigan.
• 2+ years of PTA experience in the last five years.
• Active CPR certification.
• Previous Home Health experience is a plus.

Responsibilities

• Participates in ongoing interdisciplinary assessment of the patient.
• Acts under the direction of the treating physical therapist and in accordance with the plan of care.
• Participates in development and evaluation of the plan of care with patient and caregiver.
• Provides services ordered by the physician as indicated in the plan of care.
• Assists the PT in documenting patient progress and response to treatment.
• Completes timely and accurate documentation of treatment and progress toward goals.
