Job summary

Amergis Healthcare Staffing is seeking a licensed Physical Therapist to evaluate patients and provide/ document therapy services per the approved plan of care and physician orders. You will collaborate with care teams to ensure quality outcomes.

Travel assignments across Massachusetts are available. Requirements include active PT license in state, degree from an accredited PT program, CPR if applicable, and compliance with applicable regulations.

Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• CPR certification if applicable.
• Must meet all federal, state and local requirements.
• Must be at least 18 years old.

Responsibilities

• Perform patient evaluations and provide therapy services per the plan of care and physician orders.
• Document therapy services per plan of care.

Graduate of an accredited school of Physical Therapy

Job description

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

• Current licensure as a Physical Therapist in the state of practice
• Graduate of an accredited school of Physical Therapy
• One (1) year of prior professional Physical Therapy experience preferred
• Current CPR if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
