Amergis Healthcare Staffing is seeking a licensed Physical Therapist for travel assignments in Illinois. The PT will perform patient evaluations and provide and document therapy services according to the plan of care and physician orders.

Formación

• Current licensure as a Physical Therapist in the state of practice (IL)
• Graduate of an accredited school of Physical Therapy
• One year of prior professional Physical Therapy experience preferred
• Current CPR if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

Responsabilidades

• Perform patient evaluations and assess therapy needs.
• Provide and document therapy services per plan of care and physician orders.
• Collaborate with care teams to ensure plan of care compliance.

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

• Current licensure as a Physical Therapist in the state ofpractice
