Job summary

Trinity Health At Home - Grand Rapids seeks a Home Health Physical Therapist to deliver one-on-one therapy in patients’ homes, assessing needs and crafting individualized plans while educating patients and caregivers for safe recovery.

Requires MI licensure and potential for sign-on bonus. Opportunity to impact patients’ mobility and independence with evidence-based care and collaborative clinical leadership.

Qualifications

• Current licensure as a Physical Therapist in Michigan is required.
• Graduation from an accredited physical therapy program.
• At least 1 year of clinical home care experience preferred.
• Driver’s license and reliable transportation required.
• Ability to work independently and manage time effectively.

Responsibilities
