Job summary

Trinity Health At Home - Grand Rapids is seeking a Home Health Physical Therapist to service patients in the North, South, or Greater Grand Rapids area. Full-time or part-time positions available with guaranteed hours and a patient-centered approach at home.

You will deliver high-quality, compassionate care using advanced tools and clinical expertise to help patients heal, regain independence, and thrive in the comfort of their homes.

Qualifications

• As part of the minimum qualifications, a PT must have an approved PT degree with internship.
• Licensed to practice as a physical therapist in the applicable state.
• At least 1 year of physical therapy experience or a 6–8 week preceptorship.
• Working knowledge of home healthcare guidelines and participation standards.
• Ability to function with minimal supervision and maintain professional relationships.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Accurately assesses clients at admission and other required time points.
• Inform clients/caregivers about services and patient responsibilities when therapy starts.
• Refer for evaluation/care and coordinate with other disciplines.
• Report changes in client condition to physicians and update plans of care.
• Initiate preventive, therapeutic and rehabilitative procedures per plan of care.
• Perform manual therapy tasks as established by the plan of care.

Position Highlights

• Accurately assesses clients at admission and other required time points and utilizes appropriate standardized documentation. Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.

• If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.

• Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

• Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment.

• Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients.

• Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.

Minimum Qualifications

• Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.

• Licensed to practice as a physical therapist in the state of practice.

• Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum of 6-8 weeks.

• Current working knowledge of conditions of participation and home healthcare experience preferred.

• Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with agency’s clients and families.

• Must have current Driver's license and reliable transportation to and from work site.
