Trinity Health At Home - Grand Rapids (Michigan) is seeking a Home Health Physical Therapist to provide skilled, patient-centered therapy in home settings. You will assess needs, create individualized treatment plans, and administer evidence-based interventions to promote safe, independent mobility.

You will educate patients and caregivers, support recovery at home, and collaborate with a clinical team to optimize outcomes.

Formación

• Current licensure as a Physical Therapist in the State of Michigan.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care experience preferred).
• Valid driver's license and reliable transportation.
• Ability to work independently and manage time effectively.
