Rehab at Home Healthcare Services is seeking passionate Physical Therapists to deliver high-quality care in patients' homes in Texas. The role requires a current Texas Physical Therapist license and a degree in Physical Therapy from an accredited program, with driver’s license and willingness to travel to patients’ homes.

Qualifications

• Active Texas PT license required.
• Degree in Physical Therapy from an accredited program.
• Valid Texas Driver's License.

Responsabilités

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and independence.
• Educate patients and caregivers on exercises, home programs, safety and techniques.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support service delivery.
• Monitor patient progress and update treatment plans as appropriate.
• Maintain compliance with clinical, ethical, and legal standards.

The ideal candidate for this position will have graduated from an accredited Physical Therapy program and hold a current Texas Physical Therapist license; candidates in the process of pursuing a degree in an accredited physical therapy program are welcome to apply.

Physical Therapist Qualifications:

• Current Texas license as a Physical Therapist
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Valid Texas Driver's License
• Home health experience preferred but not required
• Pediatric experience preferred but not required

Physical Therapist Role (PT):

• Perform physical therapy evaluations and re-evaluations.
