Job summary

Rehab at Home Healthcare Services is seeking a passionate Physical Therapist to deliver in-home PT services in Waxahachie, TX. The role involves evaluations, developing plans of care, and guiding patients toward mobility and independence.

Requirements include a current Texas PT license, PT degree from an accredited program, and valid Texas driver’s license. Home health experience is preferred but not required; pediatric experience is a plus. We offer mentorship and a robust benefits package.

Qualifications

• Texas PT license in good standing.
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Valid Texas Driver's License.

Responsibilities

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient’s needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety, and proper techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Maintain compliance with clinical, ethical, and legal standards.

Physical Therapist Qualifications:
