Job summary

Ultimate Therapy Services is hiring a Physical Therapist (PT) to provide rehabilitation services at Autumn Lake at Norwalk in Norwalk, CT. The position offers a full-time schedule with a pay range of $45–$50 per hour and a benefits package.

You will join a supportive clinical team focused on restoring movement, increasing strength, and helping residents reach their functional goals through evaluation, treatment, and education.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment and interpersonal skills.
• Post-acute or skilled nursing experience is a plus, but not required.
• New graduate Physical Therapists are welcome to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized plans of care.
• Provide skilled treatment addressing mobility, strength, balance, gait, transfers, and endurance.
• Establish functional goals and track resident progress throughout the rehabilitation process.
• Modify treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on exercises, mobility techniques, and fall-prevention strategies.
• Complete required clinical documentation accurately and on time.
• Coordinate care with PTAs, OTs, SLPs, nursing staff, and other healthcare professionals.
