Job summary

Ultimate Therapy Services is hiring an Occupational Therapist (OT) for a full-time position at Autumn Lake at Nellas in Elkins, West Virginia. The OT will work with residents to improve daily living skills and independence in a skilled nursing and post-acute care setting.

Responsibilities include evaluating residents, creating treatment plans, delivering interventions, documenting progress, and coordinating with the care team. WV licensure is required; new graduates are encouraged to apply.

Qualifications

• Active West Virginia Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.
• Strong assessment, communication, and organizational skills.

• Provide skilled interventions addressing activities of daily living, strength, coordination, balance, cognition, and functional performance.
• Help residents develop strategies to improve independence with everyday tasks.

• Recommend adaptive equipment and provide education on safe, effective use.
• Complete thorough and timely clinical documentation.
• Coordinate care with COTAs, PT, SLP, nursing, and other healthcare professionals.

Ultimate Therapy is hiring an Occupational Therapist (OT) to become part of the rehabilitation team at Autumn Lake at Nellas. Work directly with residents to strengthen everyday skills, improve functional abilities, and promote greater independence in a supportive skilled nursing and post-acute care environment.
