What You’ll Do

• Conduct physical therapy evaluations and create individualized treatment plans
• Provide therapy focused on gait training, balance, strengthening, endurance, mobility, and fall prevention
• Work with residents recovering from orthopedic, neurological, and post‑acute conditions
• Collaborate with interdisciplinary rehab and nursing teams to coordinate resident care
• Monitor progress and maintain accurate, timely documentation
