Job summary

Home&Care Home Health in Casper, WY is seeking a full-time Physical Therapist to deliver skilled at-home care, helping patients regain independence and improve quality of life.

You will assess patients, design personalized rehab plans, collaborate with physicians, supervise PTAs, and document progress to support physician plans.

New grads are welcome to apply and training is provided.

Qualifications

• Degree from an accredited Physical Therapy program (APTA-approved).
• Current, Wyoming PT license (active/unrestricted).
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy treatments to patients with injuries, illnesses, and functional limits to improve highest potential.
• Conduct assessments, document findings, and develop individualized treatment programs with measurable goals.
• Design and implement rehab programs aligned with physician plans and patient goals, adapting as needed.

• Collaborate with physicians and the care team to update and refine treatment plans.
• Supervise and provide clear guidance to Physical Therapy Assistants to ensure consistent, high-quality care delivery.
• Maintain timely, accurate, and thorough documentation of patient care and progress.
• Communicate effectively with patients, families, and team members, reporting progress and updating care plans as needed.
