Job summary

Casper Mountain Rehabilitation & Care Center in Casper, WY is seeking a Physical Therapist to join our in-house rehabilitation team on a full-time or PRN basis. You will assess residents, plan individualized treatment, and implement evidence-based programs to improve mobility, strength, balance, and independence.

New graduates are encouraged to apply and will receive mentorship in a collaborative clinical setting.

Qualifications

• Active Wyoming Physical Therapist license.
• In good standing with all applicable licensing boards.
• Experience in Skilled Nursing or long-term care preferred.
• Familiarity with Net Health documentation system is a plus.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents to assess mobility limitations and therapy needs.
• Develop and implement personalized treatment plans focused on strength, balance, gait, and functional mobility.
• Monitor patient progress and adjust treatment plans as needed in collaboration with the care team.
• Recommend assistive devices and interventions to enhance safety and independence.
• Educate patients, families, and caregivers on mobility strategies and fall prevention.
• Maintain accurate and timely documentation in compliance with state and federal guidelines.
• Participate in interdisciplinary meetings and care planning.
• Ensure compliance with all regulatory and facility policies.

Position Overview

As a Physical Therapist (PT), you will evaluate, plan, and implement individualized treatment programs using evidence-based practices to help residents improve mobility, strength, balance, and overall functional independence. You’ll also play a key role in patient and family education, interdisciplinary collaboration, and documentation of care.

Key Responsibilities
