Job summary

Pivot Home Health is seeking a Physical Therapist to deliver home-based therapy for adults and/or pediatrics across Colorado. You will assess, plan, and implement treatment in clients' homes, focusing on functional skills and comfort in daily activities.

Join a clinician-first team with flexible scheduling, autonomy, and ongoing professional development. Must hold CO PT license and CPR certification; previous home health or pediatric experience is a plus.

Responsibilities

• Provide home-based physical therapy to pediatric and adult clients.
• Customize treatment plans and progress notes for home settings.
• Collaborate with families and caregivers to support independence.
• Maintain documentation with iPad/electronic systems.

Region: Brighton, CO
We are hiring throughout the Front Range and encourage you to still apply if interested in other locations!
Employment Type: full-time or part-time (FT is 20 visits/week)
Compensation: $80-$90+ per regular visit
Caseload: Adults and/or pediatrics
