Job summary

Trilogy Health Services in Ohio is seeking a Physical Therapist (PT) to provide comprehensive PT services in an interdisciplinary setting. You will assess, plan, and implement therapeutic interventions, educate patients and families, and document outcomes according to facility policies.

Qualifications

• Master's degree in Physical Therapy or equivalent.
• Active PT licensure in the state; CPR certification preferred.

Responsibilities

• Provide comprehensive PT services including assessment and treatment planning.
• Implement individualized treatment plans with therapeutic exercises and modalities.
• Educate patients and families on exercise techniques and body mechanics.
• Maintain accurate documentation of patient progress and outcomes.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Perform other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
