Job summary

Trilogy Health Services is seeking a Physical Therapist to deliver comprehensive PT services in an interdisciplinary setting. You will assess, plan and implement treatments to improve patient function, with a focus on education and safe, evidence-based practices.

Requirements include a Master’s degree in Physical Therapy and current state licensure, with CPR preferred. Travel is minimal. The role offers a stable, supportive work environment within the Trilogy network.

Qualifications

• Provide full range of PT services including assessment, planning and interventions.
• Educate patients and families on exercise techniques and body mechanics.
• Maintain accurate documentation and comply with policies and regulations.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including exercises, manual therapy and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics and self-management strategies.
• Maintain accurate and up-to-date documentation of patient progress, plans and outcomes.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
