Job summary

Trilogy Health Services is seeking a Physical Therapist to provide comprehensive PT services within an interdisciplinary team. You will assess, plan, and administer rehabilitation programs to improve patient function and quality of life.

The role requires a Master’s Degree in Physical Therapy, active PT licensure, and CPR certification (preferred). Travel is minimal, with opportunities to contribute to a compassionate senior care environment.

Qualifications

• Master's degree in Physical Therapy.
• 0-1 years of experience as PT or in related field.
• Current state PT licensure or certification.
• CPR certification preferred.

Responsibilities

• Assess patients and evaluate physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of progress, plans, and outcomes per policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Perform other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
