Job summary

Trilogy Health Services in Indiana is seeking a Physical Therapist to deliver comprehensive PT services within an interdisciplinary care setting. The role includes assessment, treatment planning, and therapeutic interventions tailored to each patient’s needs.

You will educate patients and families on exercise techniques, document progress accurately, and collaborate with the care team to ensure high-quality outcomes. Travel is typically limited to facility duties as required.

Qualifications

• Master's Degree in Physical Therapy required and licensed to practice in the state.
• Provide full range of PT services, including assessment and treatment planning.
• Maintain accurate documentation and comply with policies.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises and modalities.
• Educate patients and families on exercises and body mechanics.
• Maintain up-to-date progress notes and outcomes in compliance with policies.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master DegreeExperience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
