Job summary

Trilogy Health Services is seeking a Physical Therapist to provide comprehensive PT services in a senior living setting. You will assess patients, plan and implement therapies, and educate families on safe exercise techniques. Documentation and regulatory compliance are essential in this role.

You will work within an interdisciplinary team to ensure holistic patient care, with opportunities for professional growth and a supportive company culture.

Qualifications

• Master's degree in Physical Therapy is required.
• Current valid state PT licensure or certification is required.
• CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans including exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of progress and outcomes in compliance with policies and regulations.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
