Job summary

Amergis is seeking a contracted full-time Physical Therapist in Annapolis, MD to evaluate patients and provide therapy services. The role requires documentation per the plan of care and physician orders.

Candidates must be licensed as a PT in the state and have graduated from an accredited PT program; one year of experience is preferred. CPR and health clearance may be required. 40-hour weeks with pay at $60 per hour.

Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• Graduate of an accredited school of Physical Therapy.
• One year of prior professional Physical Therapy experience preferred.
• CPR certification if applicable.
• TB Questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform patient evaluations.
• Provide and document therapy services per the plan of care and physician orders when applicable.
• Adhere to plan of care and physician orders.

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

Minimum Requirements
