Job summary

Interim HealthCare of Oakland County is seeking a Home Health Physical Therapist in Michigan to deliver patient-centered care in home settings.

You will assess mobility, plan of care, and work with a team to support goals; requires state licensure, CPR, and 2+ years in PT, preferably home health. You will educate patients and families, document progress, and adjust therapy plans while maintaining compliance with state and federal home health regulations.

Qualifications

• Graduate of an accredited Physical Therapist Program with active Michigan PT license.
• Minimum of 2 years of physical therapy experience, ideally in home health.
• CPR certification and knowledge of state/federal home health regulations.
• Strong clinical judgment and compassionate, interpersonal care.

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functional needs.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to implement plans of care.
• Observe movements, document progress, and adjust care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on plan of care, goals, exercises, and safe home exercises.
• Coach families on safety and home exercise progression.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands‑on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self‑care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Michigan
• Minimum of 2 years of physical therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
