Interim HealthCare of Oakland County is seeking a Home Health Physical Therapist in Michigan to deliver patient-centered care in home settings.
You will assess mobility, plan of care, and work with a team to support goals; requires state licensure, CPR, and 2+ years in PT, preferably home health. You will educate patients and families, document progress, and adjust therapy plans while maintaining compliance with state and federal home health regulations.
