Job summary

Interim HealthCare Home Health and Hospice in Michigan is seeking a Home Health Physical Therapist to help patients restore strength and mobility in their own homes. You’ll deliver customized therapy, collaborate with a home health team, and tailor plans of care to real daily living.

The role emphasizes 1:1 patient care, measurable outcomes, and flexible assignments with paid vacation, weekly pay, and the company’s ESOP. NEW GRADS welcome; a Michigan PT license and CPR certification are required.

Qualifications

• Graduate of an accredited Physical Therapist program with active Michigan PT license.
• Experience or willingness for home health setting and patient education.
• Strong interpersonal skills and compassionate patient care.

Responsibilities

• Provide home-based physical therapy to patients with mobility and strength loss.
• Collaborate with a home health team to implement the plan of care.
• Observe movements, assess progress, and adjust care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility.
• Educate patients and families on the plan of care and self-care.
• Coach families on therapy goals and safe exercise assistance.

As aHome HealthPhysical Therapist,here’s a big-picture view of whatyou’ll do:

• Provide home-basedphysical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patientson their plan of care,establishedgoals, exercises and self-care
• Coach family ontheplan of care, therapy goals and safelyassistingwith exercises

A few must-haves for Home Health Physical Therapists:

• Graduate ofanaccreditedPhysicalTherapistProgramand activePTlicense in Michigan(NEW GRADS WELCOME!)
• 1 year of clinical experience, preferably within home health in the last 5 years
• CPR certification
• Good clinical judgement, strong interpersonal skills,dexterityand compassion
