Job summary

Interim HealthCare Home Health and Hospice in Michigan is seeking a Home Health Physical Therapist to help patients regain strength in a home setting with personalized therapy. You will assess, design and implement individualized treatment plans to improve mobility and independence in daily life.

You’ll work with a dedicated home health team, provide education to patients and families, and monitor progress to adjust plans as needed.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in Michigan (NEW GRADS WELCOME!)
• 1 year of clinical experience, preferably within home health in the last 5 years
• CPR certification
• Good clinical judgement, strong interpersonal skills, dexterity and compassion

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength and functional challenges
• Work as part of a home health team focusing on patient plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

As aHome HealthPhysical Therapist,here’s a big-picture view of whatyou’ll do:

• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
