Job summary

Live Well Homecare is seeking a licensed Physical Therapist for outpatient pediatric care. You will evaluate children, develop treatment plans, and deliver therapy to improve functional outcomes in collaboration with families and the care team.

You will educate families, document services per policies, and supervise students or assistants as needed, contributing to discharge planning and ongoing patient progress.

Qualifications

• Must hold a Bachelor’s Degree in Physical Therapy (PT) and a current PT license.
• Master’s degree is preferred but not required.

Role Overview

Provides skilled physical therapy services in an outpatient setting, focusing on developmental and orthopedic pediatric patients. Responsible for evaluating, developing treatment plans, and delivering therapy to improve patient outcomes.

Key duties include educating patients and families, collaborating with healthcare teams, delegating tasks to assistants and students, and completing documentation aligned with departmental policies. Conducts assessments, reassessments, and consultations for physical therapy services.

• Establishes treatment plans considering discharge needs and functional outcomes.
• Communicates effectively with patients, families, and other providers, recommending referrals as needed.
• Delegates interventions appropriately based on skill levels.
• Ensures accurate documentation of services provided.

Qualifications & Skills

• Bachelor's Degree in Physical Therapy (PT); Master's degree preferred.
• Licensure as a Physical Therapist; no required experience or supervisory background.
