Job summary

LiveWell Homecare Agency seeks a licensed Physical Therapist to provide patient-centered therapy across diverse age groups. You will collaborate with an interdisciplinary team to assess needs and develop individualized care plans, applying evidence-based practices.

In this role, you’ll participate in discharge planning, mentor staff, and contribute to quality improvement while ensuring safety and cultural sensitivity in care delivery.

Qualifications

• Doctorate in Physical Therapy required and active PT licensure.
• BLS certification due within 90 days of start date.
• High School Diploma or GED equivalently acceptable.
• No prior experience required; orientation and mentorship provided.

Role Overview

Provides physical therapy treatment across diverse age groups, collaborating with an interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities

Assess patients, develop therapeutic programs, and adjust interventions based on ongoing evaluations. Effectively communicate with team members, patients, and families. Participate in discharge planning, promote evidence-based practices, and support student training and staff mentorship. Maintain departmental policies, contribute to quality improvement, and foster positive team relationships. Handle cultural diversity, seek professional feedback, and engage in continuous learning.

Qualifications

Requires a high school diploma or GED, a Doctorate in Physical Therapy, and licensure as a Physical Therapist. Basic Life Support certification is mandatory within 90 days of start. No prior experience necessary.
