Job summary

LiveWell Homecare Agency is seeking a licensed Physical Therapist to provide outpatient pediatric therapy. You will evaluate patients, develop and implement treatment plans, and deliver therapy to improve functional outcomes for children with developmental and orthopedic needs.

You will educate families, collaborate with healthcare teams, delegate tasks to assistants and students, and ensure accurate documentation according to policy.

Qualifications

• Bachelor's degree in Physical Therapy (PT) is required.
• Licensure as a Physical Therapist is required.
• Master’s degree is preferred.

Responsibilities

• Evaluate patients and develop treatment plans to improve outcomes.
• Deliver therapy in outpatient pediatric settings.
• Educate patients and families on therapy goals and progress.
• Collaborate with healthcare teams to coordinate care.
• Delegate tasks to assistants and students when appropriate.
• Complete documentation in line with departmental policies.
• Perform assessments, reassessments, and consultations for PT services.

Job description

Key duties include educating patients and families, collaborating with healthcare teams, delegating tasks to assistants and students, and completing documentation aligned with departmental policies. Conducts assessments, reassessments, and consultations for physical therapy services.

• Establishes treatment plans considering discharge needs and functional outcomes.
• Communicates effectively with patients, families, and other providers, recommending referrals as needed.
• Delegates interventions appropriately based on skill levels.
• Ensures accurate documentation of services provided.

Qualifications & Skills:

• Bachelor’s Degree in Physical Therapy (PT); Master’s degree preferred.
• Licensure as a Physical Therapist; no required experience or supervisory background.
