Job summary

Patewood Post Acute in Greenville, SC is seeking a licensed Physical Therapist Assistant (PTA) to deliver high-quality therapy under the supervision of a Physical Therapist. The role focuses on helping residents regain mobility, strength, and independence.

You will document treatments, participate in care planning, and support discharge planning and caregiver education. A SC PTA license in good standing and experience in skilled nursing or rehab are preferred; familiarity with PointClickCare or

• Current PTA license in South Carolina (required, in good standing).
• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Provide therapy treatments as directed by the Physical Therapist.
• Follow physician treatment plans and established goals.
• Document daily treatments and weekly progress notes.
• Communicate progress, concerns, and plans with the care team.
• Participate in care planning and rehabilitation conferences.
• Assist with discharge planning and caregiver education.
• Maintain clean, safe therapy areas and equipment.
