Job Summary and Qualifications

The Physical Therapist provides high-quality, patient-centered care by managing therapeutic care for assigned patients, coordinating with physicians and care teams, and developing individualized care plans.

Responsibilities include:

• Providing evaluations and treatment interventions within legal and ethical scope.
• Assessing therapeutic/functional status and developing care plans.
• Delivering therapeutic care in patients' homes and instructing patients/families on therapies and devices.
• Prioritizing caseloads and coordinating staffing and handoffs.

Qualifications:

• Basic Cardiac Life Support certification within 30 days of employment.
• Licensed Physical Therapist.
• Bachelor's Degree.
