Job summary

LaurelHurst in Portland, OR is seeking a licensed Physical Therapist for PRN weekend coverage to deliver high-quality direct patient care and comprehensive assessments.

You'll collaborate with a supportive healthcare team, maintain accurate EMR documentation (PCC), and contribute to patient rehabilitation plans while enjoying flexible hours and a bright, inclusive workplace at LaurelHurst.

Qualifications

• Licensed Physical Therapist in Oregon with current license.
• Graduated from an accredited PT program.
• Experience conducting PT assessments and developing rehab plans.
• CPR certification and ability to document in PCC EMR.

Responsibilities

• Perform PT assessments of referred patients and provide direct care.
• Develop and implement rehabilitation plans in collaboration with the care team.
• Maintain accurate PCC EMR documentation and patient progress notes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
