Job summary

Nexus Health in Mascoutah, IL is seeking an Occupational Therapist to evaluate and treat residents to help them regain, maintain, or improve daily living skills and functional independence.

You will develop personalized treatment plans, monitor progress, coordinate with the interdisciplinary team, and ensure thorough documentation meets regulatory requirements. This role offers a collaborative culture and opportunities for professional growth.

Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
