Job summary

Addus HomeCare is seeking a licensed Physical Therapist for full‑time work in a Tennessee home health setting. You’ll deliver one‑on‑one PT services in patients’ homes, helping them recover, manage chronic conditions, and maintain independence.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• At least two years of experience in an acute care or similar setting (preferred).
• Current CPR and First Aid certification.
• Reliable transportation, valid driver’s license, and current automobile insurance.

Responsibilities

• Provide physical therapy services according to a physician’s plan of care and state practice guidelines.
• Evaluate patient function and develop individualized care plans.
• Educate patients, families, and caregivers on treatment plans and therapeutic activities.
• Translate prescribed exercises into functional activities and activities of daily living.
• Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress.
• Document clinical notes, progress reports, and summaries in a timely and accurate manner.
• Maintain accurate clinical records and track patient goals and outcomes.

What You�ll Do

• Provide physical therapy services according to a physician�s plan of care and state practice guidelines

• Recommend modifications to equipment and home environments to support patient safety and independence
• Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
• Document clinical notes, progress reports, and summaries in a timely and accurate manner
• Support and motivate patients and families throughout the rehabilitation process
• Supervise Physical Therapist Assistants in accordance with company policy and state regulations
• Maintain accurate clinical records and track patient goals and outcomes

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
• Current Physical Therapist (PT) license in the state of practice
• At least two (2) years of experience in an acute care or similar setting (preferred)
• Current CPR and First Aid certification
• Reliable transportation, valid driver�s license, and current automobile insurance
