Job summary

Charming Lakes Rehabilitation is seeking a dedicated Licensed Physical Therapist to join our rehabilitation team in Lakeland, FL. This PRN role focuses on evaluating residents, creating personalized therapy plans, and guiding mobility training to help residents regain independence.

The ideal candidate has at least 2 years of PT experience (nursing home preferred), holds an active Florida PT license, and is comfortable using Rehab Optima and PointClickCare.

Qualifications

• Minimum of 2 years of Physical Therapist experience; nursing home experience preferred.
• Current, active, and unencumbered Physical Therapist license in Florida.
• Experience with Rehab Optima and PointClickCare preferred.

Responsibilities

• Evaluate residents and develop personalized treatment goals and therapy plans.
• Provide interventions to improve gait, strength, coordination, endurance and mobility.
• Design, fit and train residents in adaptive equipment, orthotics, and prosthetics.
• Collaborate with the interdisciplinary team on the best therapeutic approach.
• Instruct residents in mobility techniques, monitor progress and adjust treatment.
• Participate in care planning and resident care meetings.
• Supervise and mentor rehab staff and ensure state standards compliance.
• Conduct in-service trainings and share clinical expertise with staff.
• Maintain therapy equipment and ensure a safe, organized work environment.
