Job summary

Charming Lakes Rehabilitation in Lakeland, FL is seeking a Licensed Physical Therapist to join our compassionate rehab team. You will evaluate residents, develop treatment goals, and deliver therapy to improve gait, strength, and mobility with patient-centered care.

The ideal candidate has at least 2 years of PT experience, possesses a Florida license, and is comfortable working in a nursing home environment. Rehab Optima and PointClickCare experience is a plus, and you will mentor rehab staff.

Qualifications

• Minimum of 2 years of Physical Therapist experience; nursing home experience preferred.
• Current, active Florida PT license.
• Experience with Rehab Optima and PointClickCare preferred.
• Strong organizational and communication abilities.

Responsibilities

• Evaluate residents and develop personalized treatment goals and therapy plans.
• Provide interventions to improve gait, strength, coordination, endurance, and mobility.
• Design, fit, and train residents in adaptive equipment, orthotics, and prosthetics.
• Collaborate with interdisciplinary team to determine best therapeutic approach.
• Instruct residents in mobility techniques, monitor progress, adjust programs.
• Participate in resident care meetings and contribute to care planning.
• Supervise rehabilitation staff while ensuring compliance with standards.
• Conduct in-service training with nursing and medical staff.
• Maintain therapy equipment and keep a safe, organized workspace.
