Job summary

Charming Lakes Rehabilitation in Lakeland, FL is seeking a licensed Physical Therapist to join our rehabilitation team. This PRN role requires evaluating residents, developing personalized therapy plans, and helping residents regain independence.

The ideal candidate has at least 2 years of PT experience with nursing home exposure. Proficiency with Rehab Optima and PointClickCare is preferred; strong communication and organizational skills are essential.

Qualifications

• Current, active Florida Physical Therapist license.

Responsibilities

• Evaluate residents and develop personalized treatment goals and therapy plans.
• Provide therapeutic interventions to improve gait, strength, coordination, endurance, and mobility.
• Design, fit, and train residents in adaptive equipment, orthotics, and prosthetics.
• Collaborate with the interdisciplinary team to determine the most effective therapeutic approach.
• Instruct residents in mobility techniques, monitor progress, and adjust treatment programs as needed.
• Participate in resident care meetings and contribute to individualized care planning.
• Supervise and mentor supportive rehabilitation personnel while ensuring compliance with professional and state standards.
• Conduct in-service training sessions and share clinical expertise with nursing and medical staff.
• Maintain therapy equipment and ensure a safe, organized, and well-supplied work environment.
