Job summary

Blue Water Post-Acute is seeking a dedicated Physical Therapist to join our rehabilitation team in Florida. You will evaluate residents, develop individualized treatment plans, and implement therapies to restore mobility and independence.

You will collaborate with physicians, nurses, and other therapists, maintain clinical documentation, and participate in care plan meetings to ensure optimal resident outcomes and quality of life.

Qualifications

Help Residents Restore Mobility and Independence

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team at a skilled nursing facility in Florida. This role offers the opportunity to make a meaningful impact by helping residents regain strength, mobility, balance, and functional independence following illness, injury, surgery, or hospitalization.

As a Physical Therapist, you will work collaboratively with an interdisciplinary team to develop and implement individualized treatment plans that support optimal resident outcomes and quality of life.

What You'll Do

• Evaluate residents' physical abilities, mobility limitations, strength, balance, and functional needs.
• Develop and implement individualized treatment plans to improve mobility, function, and overall well-being.
• Provide therapeutic exercises, gait training, balance training, transfer training, and pain management interventions.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers on exercises, mobility techniques, and injury prevention strategies.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in care plan meetings and interdisciplinary team discussions.

Qualifications

• Required
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program.
• Current Florida Physical Therapist license in good standing.
• Strong assessment, treatment planning, and clinical decision-making skills.
• Excellent communication and interpersonal abilities.
• Commitment to delivering high-quality, resident-centered care.
• Preferred
• Experience in a skilled nursing facility, rehabilitation center, post-acute care, or long-term care setting.
• Knowledge of Medicare, PDPM, and rehabilitation regulations.
• Experience using electronic medical record (EMR) systems.
• Bilingual skills are a plus.
