Job summary

Ridgeway Post Acute in Petaluma, CA seeks a licensed Physical Therapist (PT) to evaluate residents, develop individualized treatment plans and deliver skilled therapy. You’ll collaborate with nursing and medical staff to restore mobility and independence in a post-acute setting.

Qualifications

• Current and unrestricted PT license in California.
• Graduate of an accredited PT program with solid clinical skills.
• Experience in SNF or post-acute rehab setting preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services per orders and care plans.
• Improve mobility, balance, strength, and functional abilities.
• Document evaluations, progress, and outcomes accurately.
• Collaborate with physicians, nursing and rehab teams.
• Educate patients and families on treatment plans and safety.
• Monitor progress and adjust programs as needed.
• Ensure compliance with state regs and facility standards.
• Participate in care planning and discharge planning.
