Job summary

Ridgeway Post Acute in Petaluma, CA seeks a dedicated Physical Therapist (PT) to join our rehabilitation team on a full-time basis. You will evaluate patients, develop individualized plans, and provide skilled therapy to improve mobility, balance, strength, and independence.

Qualifications

• Current and unrestricted Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• Ability to work effectively within a multidisciplinary healthcare team

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and established care plans
• Improve patient mobility, balance, strength, coordination, and functional abilities
• Accurately document evaluations, treatment progress, and patient outcomes
• Collaborate with physicians, nursing staff, and interdisciplinary team members
• Educate patients and families regarding treatment plans, exercises, and safety techniques
• Monitor patient progress and adjust treatment programs as needed
• Ensure compliance with state regulations, facility policies, and professional standards of practice
• Participate in care planning and discharge planning activities
