Petaluma Post Acute Rehab, a 5-Star Medicare rated Skilled Nursing community in Petaluma, California, is hiring a licensed Physical Therapist on a PRN basis to join our high-performing rehabilitation team. Qualifications

• Current, active, and unrestricted Physical Therapist license in California.
• Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate).
• Successful completion of the NPTE.
• Current CPR/BLS certification or willing to obtain per policy.
• Experience in skilled nursing/post-acute/hospital settings preferred.
• Training in rehabilitative/restorative therapy practices.
• Excellent verbal and written communication with sound clinical judgment.
• Professionalism and calm under pressure in fast-paced environments.
• Strong organization and leadership with independent clinical decision-making.
• Team-first mindset and passion for geriatric/post-acute care.

Responsabilités

• Perform comprehensive evaluations and reassessments per orders and CA standards.
• Develop, implement, and modify individualized plans of care with measurable goals.
• Deliver skilled interventions, solo and group, to restore function.
• Monitor progress and adjust interventions based on outcomes.
• Supervise PTAs and support staff per CA law and standards.
• Oversee rehab aides, students, and volunteers as assigned.
• Communicate findings and discharge outcomes to physicians and families.
• Instruct patients/caregivers in home programs and device use.
• Ensure timely, accurate documentation per requirements.
• Participate in MDS, care conferences, and care planning.
• Support discharge planning and community reintegration.
• Contribute to QA, PI, and risk management initiatives.
• Maintain HIPAA, safety, and state practice compliance.
• Treat residents with dignity, respect, and professionalism.
