Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team in Clermont, FL. You will deliver individualized, evidence-based therapy to children in their homes, with schedule flexibility and a supportive team environment.

You will evaluate patients, develop plans, and provide PT interventions to improve motor skills and developmental outcomes. The role requires FL licensure and strong collaboration with families and the clinical team.

Qualifications

Job Summary:

Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

About the Role:

As a Physical Therapist or PTA, you will work closely with our multidisciplinary team to provide high-quality, patient-centered care to patients in FL. You will evaluate patients, develop individualized treatment plans, and provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes. With a focus on pediatric patients, you will work closely with families to ensure a collaborative and effective treatment approach.

Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes

• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications:

• Active FL Physical Therapist or Physical Therapist Assistant License — required

• Basic computer proficiency for electronic documentation

• Strong clinical assessment and interpersonal communication skills

• Ability to work independently in a home health environment

• Valid FL driver's license and reliable transportation for home visits

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

Job Details:

• Job Type: Part-time, Per Diem

• Salary: $60 - $80 per hour

• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
