Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, enjoying schedule flexibility and a supportive team environment.

Responsibilities include evaluating patients, creating treatment plans, and documenting care while collaborating with the clinical team to ensure coordinated therapy. NY license, home visits, and pediatrics experience preferred.

Qualifications

• Active New York State Physical Therapy License (PT or PTA) required.
• Proficient with electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid NY driver’s license and reliable transportation for home visits.
• Pediatric experience preferred.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Job Details:

• Job Type: Full-time, Part-time, Per Diem

• Salary: $45–$70 per hour

• Schedule: Flexible — choose your own hours; day and evening shifts available, Monday–Friday, with weekend availability as needed
