Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our pediatric home health team in Monroe, NY. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, with flexible scheduling and a supportive team environment.

Requirements include NY State PT/ PTA license, ability to work independently, and experience in home health or SNF settings. Full-time, part-time or per diem options available, with day/evening shifts and a driver’s license

Qualifications

• Active New York State Physical Therapy License (PT or PTA) is required.
• Strong clinical assessment and interpersonal communication skills.
• Basic computer proficiency for electronic documentation.
• Ability to work independently in a home health environment.
• Pediatric experience preferred.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on assessment and goals.
• Provide evidence-based PT interventions to improve motor skills and functional outcomes.
• Maintain accurate documentation of patient care in regulatory compliance.

Job Summary:
Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

Job Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications:

• Active New York State Physical Therapy License (PT or PTA) — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid NY driver's license and reliable transportation for home visits
• Pediatric experience preferred
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
